Several thoughts:
1) Are you running out of disk space?
2) Are you using locks in procmail and there's a problem cropping up
with that (or the lack thereof)?
3) Do you use POP3 and delete your e-mail from the server? I've seen
a user mailbox get moved to <mailboxname>.<someextension> while
it's doing that. and if procmail tries to deliver then, and if it
doesn't have the right permissions or isn't run by postfix with
the right permissions it might not be able to create a new mailbox
file.
