Thank you for the reply's, I did manage to figure it out this morning
and although I'm a little embarrassed about it, here's the solution:

I just needed to add 
ldap_set_option($ds,LDAP_OPT_PROTOCOL_VERSION,3);
to my php page and viola.  

It actually turned out that while there weren't any errors generated,
there were warnings and I had them turned off in php...(to avoid all
those undefined variable blurbs).  Turned on the warnings and one
thing led to another...
