Hi Douglas,
I've setup a number of these gateways out there in the world, but I've
never used Bitdefender.
Mailscanner has configuration files in /etc/MailScanner/..
The main config file is MailScanner.conf. In that file is a section that
deals with Virus Scanning...
# Note: If you want to use multiple virus scanners,
# then this should be a space-separated list of
# virus scanners. For example:
# Virus Scanners = sophos f-prot mcafee
Bitdefender does show up in the list of supported virus packages, so it
should load/run it (once it knows where to find it)...
# Note: Make sure that you check that the base
# installation directory in the 3rd column of
# virus.scanners.conf matches the location you
# have installed each of your virus scanners.
# The supplied virus.scanners.conf file assumes
# the default installation locations recommended
# by each of the virus scanner installation guides.
For Bitdefender, my home install shows "/usr/local/bd7" as the place is
expects to find Bitdefender installed...
bitdefender /usr/lib/MailScanner/bitdefender-wrapper /usr/local/bd7

Once you have the config's setup properly, Mailscanner should run
Bitdefender along with ClamAV and you will see both in the processes
list.
For SpamAssassin, also check in the MailScanner.conf file to see if it
is turned on.
