Been fond of my $14.95/month TrueVoice from Earthlink. 500 minutes of long
distance
included, 911, voicemail to email, etc. Being that the wife and I have cell
phones, we
barely use the home phone, so we've never gone over the minutes.

Pros:
Good, clean calls. ((Had an echo[echo] issue 2 years ago that they fixed in
1 day.))
Voicemail recorded and sent to email. (LOVE this)
Cheap (Never had a billing issue ... unlike Vonage)

Cons:
When we moved last year, the Cisco VOIP router that they provide
had some issues when trying to dial extensions. They replaced the
box [twice] within a week and fixed the issue.
