[TriLUG] Identify this DIMM?

So I have a DIMM module and I have no idea what kind it is -- any thoughts 
on how to figure it out?

It's 84 pins, two notches (one at pin 10, the other at 40), dual sided. 
Each side has nine chips labeled Toshiba TC59S6408BFT-80 .
