 At a former employer, we used Surf Control web filtering. It was a
massive MS hosted (Win2K3) filter that did exactly what you describe;
and did it fairly well, I might add. This was for a non-profit, and we
couldn't continue to pay for the software as we had other needs. We
had to keep up and manually install the current blacklists once the
license ran out.

 I used Squid for a short time before leaving that gig; figuring that
if I had to do it manually, it would at least be an OSS tool. Squid
was tricky to set up, but it allowed me to audit every user and filter
easily.
