+1 CallCentric. They're cheap and reliable and unlike Vonage, you can hook any SIP device to them. I spend about $11 per month with them. http://www.callcentric.com/how/

Not sure what Vonage requires.
I use a Linksys PAP2 [1] (VOIP ATA) with a couple of Panasonic cordless phones plugged into it. I have also recently helped several friends purchase and setup some PAP2T devices for their home phone.
We got these for $35 from ebay [2], the only catch was to make sure the unit was not locked to Vonage. This might not be a problem for you :-)
I'm using the VOIP service from callcentric.com which has been great.
My fixed monthly cost is $3.45, and with the per minute call costs our phone comes to about $10 a month.
