What does the slow query log show? See eg. http://dev.mysql.com/doc/refman/5.1/en/slow-query-log.html

What app(s) using the database? PHP-based? If so, mod-php or cgi-based? How are database connections pooled? What tools have been put in place to to cut database load? Memcache? A reverse proxy?
