I have a book titled "PHP and MySQL Web Development" by Sams Press.  I
haven't
finished reading it all, but from what I've seen, it's a pretty decent
book.

Also, I think the "PHP4 Bible" has a section on using PHP and MySQL
together.


You can probably find both at your local neighborhood Barnes & Noble or
Borders.
