The mouse is a PS/2 Microsoft Intellimouse w/wheel.

Are you by chance using a machine with a serial mouse as opposed to PS2 or
USB?? I have had problems with several distributions of RH where the mouse
worked perfectly during install, and never after first boot when using a
serial
mouse. I had to disable gpm, then startx.
